VISION

To move Danbury forward as a community where people can age and care for others who are aging in ways that are healthy, relevant, and meaningful to individuals, cultures, and the larger community. 

MISSION

Danbury seniors will live with dignity in the place of their choice in a way that is meaningful to them. 

PURPOSE

To create opportunities for coordination and collaboration among agencies and organizations working with the growing population of older adults to identify and address service gaps in Danbury.

Medicare & Medicaid

CHOICES PROGRAM

Connecticut's program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening (CHOICES) program provides information and assistance about Medicare and other related health insurance options.

MEDICARE

Federal Insurance program that covers hospital stays and other medical expenses for people ages 65+ and people with disabilities.

MEDICARE PART D LOW INCOME SUBSIDY APPLICATIONS

Eligible for people with low income subsidies (also called Extra Help) for Medicare Part D premiums, deductibles and co-pays.

MEDICARE SAVINGS PROGRAM (MSP)

Offers financial assistance to eligible Medicare to help pay Medicare Part B premiums, deductibles and co-insurance.

HUSKY C - MEDICAID

Medicaid coverage program for low income elderly.

MONEY FOLLOWS THE PERSON

For people who have lived in nursing, chronic care, or intermediate care facilities for at least 90 days and who are on or eligible for Medicaid.

PRESCRIPTION EXPENSE ASSISTANCE

PRESCRIPTION DRUG DISCOUNT CARDS

For Veterans

BENEFITS ASSISTANCE FOR VETERANS

District offices assist veterans and their families apply for benefits and entitlements, and will advocate for Individuals when necessary.

VETERAN/MILITARY HEALTH INSURANCE

Provides preventive and primary care, offering a full range of outpatient and inpatient services for veterans.

COMMUNITY BASED OUTPATIENT CLINICS FOR VETERANS

Provides general medical care for enrolled veterans by appointment only.

ADULT CARE FOOD PROGRAMS

A federal program that subsidizes nutritious meals provided to elderly adults who participate in approved adult day care programs.

SNAP PROGRAM (FORMERLY FOOD STAMPS)

The Supplemental Nutrition Assistance Program helps eligible individuals stretch their dollars at grocery stores and farmers markets by providing monthly benefits through a credit card system.

FARMERS' MARKET NUTRITION PROGRAM

Eligible low-income seniors are provided with $18 per year to buy fruits and vegetables at authorized Connecticut farmers' markets statewide.

AARP FOUNDATION: EXPERIENCE CORPS

EC volunteers tutor students in Kindergarten through grade 3 who are struggling to learn to read. Each volunteer tutors in one participating school or after-school program 2 days a week (average of 5-6 hours) during the school year.
