CHOICES PROGRAM
Connecticut's program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening (CHOICES) program provides information and assistance about Medicare and other related health insurance options.
MEDICARE
Federal Insurance program that covers hospital stays and other medical expenses for people ages 65+ and people with disabilities.
MEDICARE PART D LOW INCOME SUBSIDY APPLICATIONS
Eligible for people with low income subsidies (also called Extra Help) for Medicare Part D premiums, deductibles and co-pays.
MEDICARE SAVINGS PROGRAM (MSP)

A federal program that subsidizes nutritious meals provided to elderly adults who participate in approved adult day care programs.
SNAP PROGRAM (FORMERLY FOOD STAMPS)
The Supplemental Nutrition Assistance Program helps eligible individuals stretch their dollars at grocery stores and farmers markets by providing monthly benefits through a credit card system.
FARMERS' MARKET NUTRITION PROGRAM
